Advances in breast imaging offer the promise for improved, early diagnosis and successful treatment. Breast imaging is a critical component of surgical planning for breast disorders, but with many available options and new emerging technologies, the complexity and sometimes conflicting nature of available information makes it difficult to determine the best course of treatment. Practitioners of evidence-based medicine (EBM) integrate individual clinical expertise with the best available clinically relevant evidence from research. The cornerstone of EBM is the systematic review, which uses rigorous transparent methods to identify, evaluate, and synthesize the results of relevant primary research. This article will identify and summarize the best available research in breast imaging from systematic reviews and additional primary research. It will address breast imaging modalities for monitoring early response to neoadjuvant chemotherapy (NAC), predicting patient response to NAC (ie, the ability of imaging to distinguish responders and nonresponders to NAC prior to initiating NAC), and improving the detection of implant rupture.
